Explanation of Forms
League Renewal Form - Each year, all leagues must renew their affiliation with NCASA/USASA by completing this form and paying a $25 fee to NCASA. The form requires contact information for the League's Board, officers, assignor, and others, along with a general demographic of the league: # of team, divisions, fields, etc.
Payment Form - With each batch of registrations submitted to the State Office, a Payment Form MUST be included to reconcile payment with registrations. Without this form, the registration process at the State Office takes longer with reconciliation. It's easy and helps simplify the process for all.
League Status Form - This form is intended to keep track of the total number of teams that register with your league. As teams are added you need only add them to the list with the appropriate designation.
