TEAM REGISTRATION & ROSTER FORM

ENTRY FEE $ 400.00 per team
PERFORMANCE / TRASH BOND $ 100.00 The bond will be returned in full by mail within 30 days after the tournament, barring any violation of State Cup rules.
ENTRY DEADLINE October 1st, 2004 (Postmarked) - NOW CLOSED. $ 25.00 late fee applied after the 1st, if accepted

DIVISIONS Men’s Open Men’s Over 30 Men’s Over 40 Men’s Over 50 Women’s Open Coed (a minimum 5 female players on the field at all times) The tournament committee reserves the right to close a division due to lack of team registration. The teams registered will receive a full refund.
SCHEDULE Teams will be guaranteed 3 matches and schedules will be posted online after entry deadline and draw. Friday evening games may be scheduled for interested teams.
ELIGIBILITY Teams must be registered and in good standing with the NCASA. Players must possess their current NCASA player pass. Guest players are allowed; refer to the rules for roster requirements.
REGISTRATION Fill out the NCASA Team and Player Registration Form and return it with the Entry Fee and Performance Bond to the North Carolina Adult Soccer Association office postmarked by the entry deadline: October 1st, 2004.
