New Player Accident Insurance from US Adult
Soccer Association
Dear
Association,
Recently, you
may have received an email from NC Adult Soccer
informing you about a new US Adult Soccer
Association (USASA) insurance program.
USASA was in a renewal year starting September
1, 2008. The USASA Board of Directors
voted on and approved a new insurance program
for the next 3 years with AIG. NCASA
continues to update this website with
information and documents related to the new
insurance program as we received relevant
documents from USASA and AIG.
Through the
links above you will finds copies of claim form
instructions in both English and Spanish
depending on the injury/claim date. These
document will be immensely helpful in assisting
you to complete the claims forms correctly.
NCASA is not
permitted to post the claim form on-line, so
you'll need to email us to
regular mail or
email you a
copy of the claim form. Be Sure to include the
following information in your email textbox:
- - Your
Name as printed on the team roster
- - Your
Player Pass Number
- - Mailing
Address, if requesting regular mail
- - Injury
Date, so we know which claim form to send
Copies of the
injury claim form have also been sent to every
local league administrator as well as most of
the team contacts, so you can request a copy
from those individuals. The procedure for
filing a claim is the same, just with a
different company and contact person.
