International
Clearance Form - [PDF]
No league shall permit a
player to play until it has received a properly
completed registration and registration fee, and
has confirmation of clearance from the
Federation Registrar, for a player who was last
registered with a team from a foreign National
Association.
This is the required form for clearance.
Amateur
Reinstatement Form - [PDF]
A professional player
who is currently registered as a professional
player with any club can apply to the Federation
to be reinstated as an amateur only with the
permission of that club and the Adult State
Association or Professional League with which
that club is affiliated.
This form must be submitted to the
Secretary General of the USSF.
